  • Information that you provide by filling in a form or forms on our website. This includes information provided at the time of registering to use our service and may include information provided to us by a third party/referring agency.
  • Records and copies of your correspondence (including e-mail addresses) if you contact us.
  • Your responses to surveys or feedback that we might ask you to complete for research purposes.

Here’s what we may do with your personal information

  • Our receptionist, center manager and team leader receive a copy of forms submitted on the website. 
  • Information you provide will be stored using Cognito Forms. (Cognito Forms is hosted securely on the Microsoft Azure cloud platform, which is PCI (DSS) Level 1 and HIPAA compliant and uses TLS 1.2/SSL encryption which is always accessed over HTTPS 100% of the time.)
  • We may use information collected from you as non-identifiable, anonymous data for reporting, statistical and funding purposes.
  • Your personal information may be passed on to the counsellor or facilitator of the service you are requesting.
  • We may contact referring agencies regarding their referral to our service.

We do not share information with any third party service without your permission.
